Universal Peace Foundation of North America, California and or its affiliates (UPFNA) provide various holistic and integrative therapeutic treatments, or wellness therapies (“Wellness Therapy”) to Participants (“Participant”). The Participant understands and acknowledges that: 1. Wellness Therapy programs offer personalized therapeutic treatments that complement a healthy lifestyle and may address physical and emotional health issues. Wellness Therapy modalities offered by UPFNA have their limitations and are provided to support traditional medical treatments or as reference only. The Participant is hereby advised there is no guarantee about the therapeutic results or accuracy of any analysis, and UPFNA cannot be held responsible in any way for any adverse consequences. 2. The Participant’s use of any information, services, or material provided by UPFNA is entirely at the Participants’ own risk, for which UPFNA shall not be liable. 3. The Participant is responsible for their own life choices and decisions. UPFNA is not qualified to give legal, financial, medical, psychological, psychiatric, or any other specialist advice. If you require such advice, you should seek the services of a licensed professional. 4. The information and material provided by UPFNA expressly exclude liability for any such inaccuracies and errors. The Participant is hereby advised to exercise common sense and their own judgment while employing the therapies or interpreting any information given. I (the Participant) hereby declare and agree that: 1. Assumption of risk and release – I am aware of the inherent risk associated with employing and practicing any Wellness Therapy by UPFNA and I am willing to take the risk. I hereby release UPFNA and its associates and affiliates from, and against any and all monetary claims, liabilities, demands, actions, cause of action(s), cost and expenses whatsoever, at law or in equity, arising out of my participation in the Wellness Therapy as provided. 2. Covenant not to sue – I promise not to sue UPFNA should I suffer from any damage, loss, personal injury, or death as a result of the participation in any Wellness Therapy. 3. Waiver of liability – I waive the right to hold UPFNA liable for any liability, cost, damages that may arise from my participation in any Wellness Therapy. 4. Indemnification – I will indemnify UPFNA for any claims, causes of action, damages, judgment, cost, or expenses against UPFNA which may arise out of my participation in any Wellness Therapy.
